Incomplete Grade Request Form

Steps for submitting an incomplete grade request:

  • Student completes Section 1, signs the section, and submits the form to the instructor.
  • Instructor completes Section 2, signs the form, and uploads a copy of the completed form to BlueStar.
  • Both student and instructor should retain copies of the completed form.
Fill out the Incomplete Grade Request form
